Please download and print out each form listed below.  Bring the completed paperwork to orientation.

• • I-9 – Please bring the necessary identification documents to orientation.
  • W-4 – Federal income tax withholding form
  • OR W-4 -state income tax withholding form
  • Oregon Food Handler Card – All Ecola Part Time staff must have a current Oregon Food Handler card.
